Patient navigation
Healthcare systems can be complex and overwhelming. We walk alongside clients, assisting with appointment coordination, understanding care plans, and advocating for adequate medical support.
We help clients prepare questions for providers, understand medical information in plain language, and feel more confident participating in decisions about their care. By encouraging open communication between clients, families, and healthcare professionals, we help ensure each person’s needs, preferences, and concerns are heard.
Public Health Education
Our Public Health Education Workshops offer clear, practical guidance that empowers individuals and communities to make informed health decisions. Topics may include disease prevention, fall safety, nutrition, physical activity, mental health, vaccination, hygiene, chronic disease management, and access to local health resources.
Led by qualified educators and community partners, these workshops foster questions, discussion, and realistic steps participants can incorporate into daily life.
